INSTRUCTIONS: Please click on the below links to download the registration forms for the 2008-09/5768-69 Hebrew School Year. Print out the forms, fill out, and mail back to the OZ office. Please fill out a separate form per child. If you have any questions, please feel free to contact Barbara in the OZ office, (802) 864-0218, ext. 21, or Rabbi David Steinberg, ext. 25. You will need to have Adobe Reader installed on your computer to read/download these forms.

1. Hebrew School Registration Form for the 2008-09/5768-69 school year. Please fill out a separate registration form for each of your children in the Hebrew School - with the exception that you only need fill out the parent volunteer sign-up section once. Please return the completed Registration form to us as soon as possible.

2. Hebrew School Tuition Calculation Form for 2008-09/5768-69. Please complete the form to determine the amount of tuition for your household and return it with your check as soon as possible, preferably with your completed Hebrew School Registration Form(s). Please note that a discount of $25 per child is available if you submit your completed registration forms and payment by July 1. Also please note: If you need to request need-based scholarship assistance and/or to arrange a payment plan, please contact the synagogue's Executive Director, Mike Kanarick, at 864-0218 x 26. All such inquiries and arrangements are conducted in strict confidence. (Need-based scholarship assistance is only available to OZ members).
